What the New Data on Women Actually Show

The study driving the current wave of headlines examined a national sample of adult American women and measured biological aging using DunedinPACE, an epigenetic clock that estimates the pace of physiological decline from patterns of DNA methylation — chemical tags that accumulate on the genome as cells age and stress accumulates. Researchers found that women reporting greater “declining health anxiety,” the fear of losing physical capacity as they age, showed a 0.07 standard-deviation increase in DunedinPACE scores, a small but statistically real association. Cumulative aging anxiety overall produced a similar effect size — but that association weakened once the researchers adjusted for other variables, a detail that matters more than the headline number.

DunedinPACE and its cousins — PhenoAge, GrimAge, the Klemera-Doubal biological age — do not measure age in years. They measure the rate at which a person’s cells and organ systems are functionally deteriorating relative to peers of the same chronological age. Two people born the same year can carry meaningfully different biological ages, and that gap has been linked in prior research to mortality, chronic disease risk, and cognitive decline. It is a genuinely useful tool. But a 0.07 standard-deviation shift is a small effect in absolute terms, the kind that shows up reliably only in large samples and tells you little about any individual woman’s trajectory.

A Concept With a Long History: Why Aging Anxiety Is Gendered

Aging anxiety is not a new construct. Psychologists have studied it for decades, and the literature has consistently found that women report it more acutely than men, driven less by fear of death itself than by anticipated loss of physical attractiveness, health, and independence. Sociological work on “multiple old ages” has shown that how a woman experiences aging depends heavily on social context — income, marital status, cultural attitudes toward appearance — meaning aging anxiety was never a purely internal psychological state but one shaped by external pressure. This history matters because it means today’s biological-aging findings are landing on top of a well-documented, pre-existing gender gap in how aging is feared and experienced, not introducing a new phenomenon.

The Causality Problem Nobody’s Headline Mentions

The central weakness in the “anxiety ages you” narrative is direction. A systematic review pooling multiple studies on anxiety, depression, and biological aging found that greater anxiety was often associated with older biological age markers — but only half of those associations reached statistical significance, and when researchers tracked people over time rather than snapshotting them once, the anxiety-aging link disappeared entirely. That is a critical distinction: cross-sectional correlation is common in this field; durable, longitudinal causation is not.

More strikingly, large cohort studies point the arrow the opposite way. Research published in Nature Communications followed participants for a median of 8.7 years and found that people who were already biologically older faced a meaningfully higher risk of developing depression or anxiety afterward — a 5.9% increase in risk per standard deviation of accelerated biological aging by one measure, and 11.3% by another. A related analysis from Columbia and Peking University researchers reached a similar conclusion: biological aging processes appear to contribute to depression and anxiety risk, not merely result from it. Put plainly, the body may be aging the mind’s mood as much as the mind’s mood ages the body — and disentangling which comes first, in any given person, is not something a correlational study can do.

Where Menopause Complicates the Picture

For women specifically, there is a confound that any serious reading of this research has to reckon with: menopause. The hormonal transition of perimenopause and postmenopause independently raises rates of anxiety, depression, and even panic symptoms, largely through estrogen fluctuation’s effects on mood-regulating brain circuits. Because menopause coincides with midlife — precisely when aging anxiety and biological aging measures both tend to shift — any study of women’s aging anxiety that doesn’t fully control for menopausal status risks attributing to psychology what is partly a hormonal event. This doesn’t invalidate the findings, but it means the “anxiety causes faster aging” story is competing with a simpler explanation: a biological transition is driving both the anxiety and the aging markers simultaneously.
