A chemical hanging in your closet after a trip to the dry cleaner may be doing more than freshening your shirts, new research suggests it could be scarring your liver from the inside out.

Researchers led by Yinan Su, Jennifer Dodge, and Brian Lee looked at data from the National Health and Nutrition Examination Survey, a large government health check-up program covering thousands of Americans. They focused on adults tested between 2017 and 2020 who had both blood PCE readings and a liver scan called transient elastography, which measures scarring without surgery.
Out of 1,614 adults studied, people with any detectable PCE in their blood had roughly three times the odds of significant liver fibrosis compared to those with none. The exact number reported was an adjusted odds ratio of 3.17, with a statistical range running from about 1.23 to 8.20, meaning the true risk could be modest or quite large, but is very unlikely to be zero.
What makes the finding harder to dismiss is the dose pattern. Researchers found that for every one nanogram per milliliter rise in blood PCE, the odds of fibrosis jumped even further, a dose-response relationship that scientists treat as a stronger signal than a simple yes-or-no exposure comparison.
Why Scientists Ran A Negative Control Check
To guard against a fluke result, the team ran what is called a negative control analysis, essentially testing whether the same statistical pattern showed up for an exposure that should have nothing to do with liver scarring. The association held up under that check, which is one reason the authors argue this is not just noise in the data.
Doctors at Keck Medicine of University of Southern California, who worked on the study, noted the scarring showed up even in people without the usual suspects like heavy drinking or obesity driving the damage. That detail matters because it points to PCE as a possible independent contributor, not just a bystander riding along with other risk factors.
Liver fibrosis is not a small concern. It is a buildup of scar tissue that, left unchecked, can progress toward cirrhosis, liver failure, or cancer. Roughly seven percent of study participants were classified as having significant fibrosis, a reminder that silent liver damage is more common than many people assume, often showing no symptoms until it is advanced.
PCE Has A Long, Documented History Of Concern
This is not the chemical’s first appearance in health research. Government toxicology reviews have flagged tetrachloroethylene for hepatic and renal toxicity for decades, and international cancer researchers have classified dry cleaning exposures as possibly carcinogenic to humans. Older studies of dry cleaning workers found excess liver cancer risk among women in the trade, adding weight to concerns that go beyond this single new paper.

None of this means a trip to the dry cleaner is a health emergency. Occupational exposure, where workers breathe concentrated fumes for years, is a different risk level than an occasional whiff from a plastic garment bag. But the researchers argue this overlooked exposure deserves a seat at the table alongside alcohol, obesity, and viral hepatitis when doctors search for the roots of unexplained liver disease.
